Optimum Mobile Device Protection is a combination of insurance, service contract, and technical support.  The insurance is underwritten by Allianz Global Risks US Insurance Company; Asurion Insurance Services, Inc. (in California, Agent license #0B35141; in Iowa, Agent license #1001000131) is the Agent and provides claims servicing under this program. The service contract is provided by Asurion Technology Services, Inc., or one of its affiliates. The technical support is provided by Asurion Technology Services, Inc. and Asurion Mobile Applications, LLC.

1 Excluding Sunday and available in select markets only. This benefit is provided by the insurance program and the service contract program.

2 Cracked screen repair is available for select devices subject to parts, location, and technician availability. This benefit is provided by the service contract program.

3 Available for eligible devices outside of the manufacturer’s warranty period that power on but fail to hold an adequate charge after diagnostic testing. Available for select devices in select areas. This benefit is provided by the service contract program.
